Message to Junior Physicians Interested in Department of Preventive Cardiology

Specially Appointed Director, Department of Preventive Cardiology
Yoshihiro Kokubo, MD, PhD, FACC, FAHA, FESC, FESO

Those who have completed the initial training but have not yet designed their own future, those who have decided their own future but are not confident that this is all right, will have no end to their worries when going on a professional path from now on.
The French philosopher Alain began in a passage of "Human"with "Descartes who lost his way in a forest knows no reason to take his way more than this way,"which preaches the methodology of life. At first, you are free to choose a path, focus all your nerves on each step of your walk, and finally, when you get out of the forest as if you were choosing that path from the beginning: "It saves accidental choices and makes them good,"he concludes. You may be wondering if this path was good, but you can make your own choices and end up with the result as if you were choosing the best path from the beginning.

The "Health Examination / Health Guidance Course"is not just a medical examination and health guidance course. Medical examinations, especially consultations, are medical diagnostics themselves. Medical examinations are required to find as many diseases as possible within a limited number of tests. You can use your five senses to learn the appropriate interview, such as inspection, auscultation, and palpation. As with health guidance, it is not easy to improve lifestyle habits with all-out advice. Using the "Lifelong Health Support 10"that we have developed, you can objectively and easily find out your problems and give guidance without difficulty. At the medical examination department, you can learn internal medicine diagnostics and health guidance methods that are not embarrassing, no matter which department you go to.

On the research side, we have produced primary research results in the Suita study and advanced cardiovascular diseases risk scores that is the culmination of these results. Furthermore, using the results so far, we have implemented a heart failure aggravation prevention project and returned the research results to the citizens, and we are further spreading the results of our studies to Okinawa Prefecture. You can learn the development of such research results. We have various cohorts for those who want to master social medicine properly. Maintaining a cohort is very fragile, as if you were dealing with delicate glasswork. We are confident that you will have a valuable experience engaging in full-scale cohort studies and mastering them.

Despite Japan's highest aging rate globally, there is very little evidence for preventing organ endings. This is because epidemiological studies of cardiovascular disease are discontinued when stroke and myocardial infarction occur, and they are not tracked down to heart failure and dementia, which are the culmination of various diseases. We have launched a cohort of heart failure and dementia for the local population, even among the corona sicknesses. We are about to conduct a baseline survey. We will also start investigating the epidemiology of hearing, sight, smell, and oral hygiene, which are said to be risk factors for both diseases but have little evidence. From this, you may understand why there are fewer valve replacements in Japan than in Europe and the United States. The risk factors for each type of valvular disease and transient atrial fibrillation will be known for the first time in Japan.

We will provide completely new health guidance using Lifelong Health Support 10 for both general health guidance and health guidance to prevent aggravation of heart failure. Through this health guidance, you can learn health guidance methods that lead to the prevention of not only cardiovascular disease but also cancer and dementia. We are preparing to start non-drug intervention research with extremely little evidence in Japan. We are also considering health support from children to long-term care so that lifelong health support will be possible through these. Even doctors thinking of majoring in doctors who also have other departments can prepare courses as flexibly as possible according to their wishes. Even if your future course is lost in the woods, you will find yourself out of the woods by the time you finish your major. We hope that you will make great use of it and spread your wings around the world.

Health examination / health guidance course (resident course)

Overview

In the Department of Preventive Cardiology, you will learn medical examinations (internal medicine diagnosis) and guidance methods for lifestyle-related improvement through medical examinations (specific medical examinations, longevity medical examinations) and health guidance (active support, motivational support). It cannot be prevented by undergoing a medical examination. First of all, we will undergo a medical examination, list the problems, and provide health guidance according to the problems. The method of improving lifestyle habits is not vague guidance such as eating in a well-balanced manner and exercising, but using "Lifelong Health Support 10", each examinee can objectively understand which item does not reach, so the part that does not reach Ask the person to choose which item to improve from, and learn how to provide guidance and support for it.

Since 1989, the Department of Preventive Cardiology has been conducting the Suita Study (cohort study) for urban residents. You will learn the basic research design of epidemiology, "descriptive epidemiology", "cross-sectional study", "case-control study", "follow-up study", and "intervention study". Since we are conducting joint research with other clinical departments and research institutes within the center and institutes outside the center, we will learn the setting of research hypotheses for each research design. Through the Suita research, you will learn about the tracking method.

We have also just launched a new cohort with heart failure and dementia as endpoints. A baseline survey will be conducted over three years. We will examine cases of echocardiography and carotid echocardiography. A cognitive function test will be performed, and for suspected cases, a brain MRI test will be performed to examine the case. Learn about other cohort studies while watching the progress of your major.

If you wish to obtain a degree, please contact us as it can be obtained by setting a collaborative graduate school and meeting the prescribed criteria. We also flexibly support curriculums that combine low tension with other departments, so please feel free to contact us.

Period: 3 years (Consultation required: A curriculum tailored to rotation with other clinical departments is also available) Target: 3 years after graduating from a doctor (after initial clinical training)

Training content

1) Clinical

Through medical examinations (specific medical examinations, longevity medical examinations) and health guidance (active support, motivational support), you will learn medical examinations (internal medicine diagnosis) and lifestyle-related improvement guidance methods.

We are conducting a cohort study in line with the medical examination. Examination items include ultrasonography (echocardiography, carotid artery echo, abdominal echo), Holter electrocardiography, limb sphygmomanometer, brain MRI, and cognitive function test. In addition, we also perform audiometry (standard pure-tone hearing test, speech hearing test), fundus examination, color vision examination, intraocular pressure examination, optical interference tomography examination, and dental examination. You will learn how to read the results and explain the results to the examinees.

Case studies will be conducted for medical examinations and health guidance. In addition, we will examine cases for imaging tests. A briefing (pre-meeting) will be held before the start of the medical examination.

2) Research

You will learn about the Suita study (cohort study) and non-drug intervention research that is carried out in the medical examination (specific medical examination, longevity medical examination) and health guidance (active and motivational supports). Since we are conducting joint research with clinical research institutes and research institutes outside the center, we will learn various epidemiological studies. The tutor system is used to guide research, and small goals are set so that even the first research can naturally produce results naturally. We will teach you how to make a protocol, research background, hypothesis setting, and method. The following epidemiological studies are available for research. Other than that, it is possible to launch a new epidemiological study. Regarding presentations at academic conferences, we will first make presentations when we have a prospect of taking shape from a small place.

  • The Suita Study NEXT (September 2021- )
  • Co-creation place formation support program
  • Heart Failure Severity Prevention Measures Project (October 2020-)
  • The Suita Study (April 1989-)
  • Japan Prospective Studies Collaboration for Aging and Dementia (JPSC-AD study: April 2015-)
  • The Japan Public Health Center Cohort Study (April 1989-)

After consulting with the person himself / herself, we will make a research plan and protocol based on the field of interest and the content related to the research question and will also guide you on the research plan for ethical review. We will teach you how to write a treatise that is easy to be accepted. Participation only in cohort studies from other departments is also welcome. We will proceed after consultation.

Main schedule etc.

  • Outpatient examination of the medical examination department and explanation of results: 1-2 days a week
  • Case study meeting: About once a week
  • Echocardiography / cervical echo: Case study meeting once a week
  • Tutor (small group instruction) once a week
  • Health Examination Department Subcommittee: Once a week
  • Internal medicine group meeting in the center: Once a week
  • Medical examination at neighboring local governments: Once a month (half a day)

Health examination / health guidance course (specialized training doctor course) Overview

Overview

In the Department of Preventive Cardiology, you will learn outpatient (internal medicine diagnostics) and lifestyle-related improvement guidance methods through medical examinations (general medical examinations, longevity medical examinations for prevention of heart failure) and health guidance (active and motivational support).

Learn how to maintain quality control while having a conference with others. We will list the problems based on the medical examination results and make a plan on how to proceed with health guidance according to the problems. As for the method of improving lifestyle habits, each examinee objectively observes which item does not reach by using "Lifelong Health Support 10,"and asks the person himself/herself which item to improve from the part that does not reach. You will learn how to make choices and provide guidance and support for them.

Since 1989, the Health Examination Department has been conducting the Suita Study (cohort study) for urban residents. Furthermore, for those who wish to learn social medicine in earnest, it is possible to learn how to maintain cohort studies and coordinate with the government, medical associations, and citizens.

We have also just launched a new cohort with heart failure and dementia as endpoints. A baseline survey will be conducted over a three-year period. We will examine cases of echocardiography and carotid echocardiography. A cognitive function test will be performed, and for suspected cases, a brain MRI test will be performed to examine the case. Specialized practitioners will also learn about the quality control of each test. On the research side, we will provide tutorial guidance while observing the progress.

If you wish to obtain a degree, please contact us as it can be obtained by setting a collaborative graduate school and meeting the prescribed criteria. We also flexibly support curriculums that combine low tension with other departments, so please feel free to contact us.

Period: 2 years (Consultation required: Curriculum tailored to rotation with other clinical departments is also available)
Target: 5 years after graduating from a doctor (resident graduation level, medical school graduate school equivalent level)
