To refer patients

Please fill out the "Request for appointment/treatment information sheet"and fax it to the Office for Coordination of Specialized Medical Care (06-6833-5126), addressed to the Valvular Disease Clinic. Give your referral letter to your patient and instruct him or her to visit the clinic during the specified hours on Mondays, Wednesdays, or Fridays. Ask them to stop by Counter No. 3.

For patients with referrals

Please come to Counter No. 3, which has a sign for "New patients."Bring the referral letter. Tell the receptionist that you are visiting the Valvular Disease Clinic. Depending on the referral letter and time and the day of the week of your visit, you may be asked to come to the outpatient clinic for an initial visit.
