研究活動の概要

【世界初の脳動脈瘤治療用カバードステント開発】
　脳神経外科では、生体医工学部の中山泰秀室長と当科の佐藤徹医長が中心になって、世界に先駆け治療困難な大型脳動脈瘤の根治を目指した新規脳動脈瘤治療用ステント「多孔化カバードステント（NCVC-CS1）」を開発し、2018年度に医師主導治験を完了した。現在、実用化に向けた最終手続きを進めている。

【脳梗塞急性期血栓回収療法に関する研究】
　近年、脳梗塞急性期における血管内手技を用いた血栓回収療法のエビデンスが構築された。
当科でも、脳血管内科と合同で多数の血栓回収療法を実施している。発症後短時間であってもリスクの高い症例や、逆に6時間以上でも適応となる症例が存在すると考えられる。当科では脳血管内科、脳神経内科と共同で広範囲なDWI-Clinicalミスマッチのある症例に対する血栓回収療法の検討を続け、治療が奏功する症例群の適切な抽出法に関して、積極的な対外発信を続けている。

【もやもや病の病態と治療に関する研究】
　本法における出血型もやもや病についての他施設共同研究（JAM Trial）において、主導的な役割を担ってきた。2018年はもやもや病における「再出血発作」に、脈絡叢動脈の側副血行路増生が有意に関連することを見いだしたが、2019年は同血行路が「初回出血」の有意は危険因子であることを見いだし、論文報告を行った。また、自施設のもやもや病症例において、小児虚血型、成人虚血型、成人出血型の側副血行路増生パターンを解析し、新規知見を見いだして全国学会において発表した（論文投稿中）。

【甲状腺疾患と頭蓋内血管狭窄に関する研究】
　頭蓋内血管狭窄患者の中に、慢性甲状腺炎（橋本病）の原因となる抗甲状腺抗体陽性の頻度が高いことを見いだし、これら抗体陽性患者の頭蓋内血管形態を詳細にパターン分類し、その臨床的予後について世界初の知見を見いだして全国学会において発表した（論文投稿中）。

【その他の臨床研究】
　上記研究以外にも、高難度疾患の代表である脳動静脈奇形治療に関する研究、重症脳卒中後の血糖変動に関する研究、脳動脈瘤バイパス併用術後や脳虚血病変に対するバイパス術後の血流解析、硬膜動静脈瘻のシャントポイントを同定する新しい画像診断技術の開発など、多岐に渡る臨床研究が進行中である。

　臨床面では、全国から紹介される脳血管障害症例に対して、開頭直達手術、血管内治療、ガンマナイフを適切に使い分け、あるいは組み合わせた高水準の治療を行っている。
複雑な脳動脈瘤や特殊な頚動脈狭窄症に対しては、ハイブリッド手術室を用いた直達・血管内同時治療を積極的に行い、対外的に報告を続けている。
2019年7月の新病院移転後は脳外科専用バイプレーンハイブリッド手術室が稼動開始し、直達手術と血管内治療の複合治療に関してさらに新しい知見を蓄積している。
また新病院ではガンマナイフが最新型（ICON）に更新され、より低侵襲で効果的な治療が可能となった。これらは今後、学会・論文で積極的に報告していく予定である。

2019年の主な研究成果

　治療困難な大型脳動脈瘤の治療を可能にする多孔化カバードステント（NCVC-CS1）の医師主導治験を完了し、承認に向けて作業を行っている。

　もやもや病における新規出血発作に、脈絡叢動脈からの側副血行路増生が有意に関連することを見いだし、論文報告を行った。また、小児虚血型と成人虚血型、成人出血型間での側副血行のパターンの類似性と相違性を初めて見いだし、学会報告・論文投稿している。
また日本人に多い橋本病における頭蓋内血管狭窄に関して世界初の知見を見いだし、学会報告するとともに論文投稿中である。

　直達手術424件、血管内治療203件、ガンマナイフ治療198件の計825件の脳外科的治療を行った。
全国から紹介される治療困難な脳動脈瘤に対して直達手術と血管内治療の適切な選択を行い、また必要に応じてハイブリッド手術室を用いた同時複合治療を行っている。高難度疾患の代表である脳動静脈奇形に対して専門外来を開設し、直達手術、血管内治療、ガンマナイフ治療の3つを複合させた治療を推し進めている。厚生労働省指定難病であるもやもや病、およびこれに関連した類もやもや病の外科治療件数（2019年：73件）は国内最多レベルを維持している。豊富な症例数をもとに、さらなる病態解明と新たな治療選択基準の確立を目指した臨床研究を推進していく。

　脳動脈瘤、脳動脈解離、脳動静脈奇形、頚動脈狭窄症、急性期主幹動脈閉塞の治療方針と成績につき、脳血管障害のトップランナーとして積極的な研究成果の発表を行っている。
